ROSTER & RELEASE OF LIABILITY
I, or we as well as the undersigned participants do hereby agree to release, hold harmless and forever give up any claim against the City of Carson that we may have or may arise in the future, for any damages on account of bodily injury or property damages arising in any manner out of participation in the City of Carson Recreation Program. I understand that, should I be injured I cannot hold the City of Carson responsible and I understand that by signing this form, I have waived my legal right to attempt to hold the City of Carson responsible.
